• Please upload your insurance card ( front and back) in the client portal. 
• For certain insurance carriers we may utilize headway or ALMA to file your claim. 

It is important to confirm your insurance benefits prior to scheduling an appointment with us. As part of your responsibility, please ensure that you are aware of any out-of-pocket fees associated with your insurance plan. If we file a claim on your behalf and it is not paid by your insurance, you will be held responsible for the bill.

Streamline The Process To Avoid Any Inconvenience

We collect all copays and coinsurance fees the night before your scheduled appointment. If your payment does not go through, your appointment will regrettably be cancelled. Please understand that, due to our contractual obligations with your insurance company, we are unable to waive copays, deductibles, or co-insurance fees.
