How Do I Become A New Patient?

To join our practice, simply click the button below and schedule an appointment as a NEW PATIENT. Once you are scheduled, you'll receive a link to the client portal where you can easily complete all required intake forms and schedule your first appointment hassle-free!


It is important to complete all of the intake forms 72 hours prior to your first appointment or your appointment may be cancelled or rescheduled.

Our Goal Is To Provide The Best Mental Health Care

We are committed to providing you with the highest quality of care, tailored to your individual needs and circumstances. As part of our dedication to your health and well-being, we kindly request that all new patients have or be willing to obtain a physical exam if they have not had one within the past year, along with necessary laboratory tests. Additionally, you may be required to have an Electrocardiogram , depending on specific medications.


Physical exams and laboratory tests are essential components of preventive healthcare and enable us to assess your overall health, identify any underlying medical conditions, and develop an appropriate care plan tailored to your specific needs. These measures also help us to ensure your safety and well-being while under our care.

If you have recently undergone a physical exam and had laboratory tests conducted within the past year, please provide us with the relevant documentation or let us know the details so that we may update your records accordingly.


For those who have not had a physical exam or recent laboratory tests, we encourage you to schedule these appointments at your earliest convenience. If you need assistance finding a healthcare provider or arranging for laboratory tests, please don't hesitate to reach out to us. We are here to support you every step of the way.

A Release of Medical Records

We are dedicated to providing you with the highest quality of care and ensuring that your healthcare journey is as seamless and effective as possible. In order to facilitate collaboration and continuity of care, we kindly request that you complete a Release of Information (ROI) form for your primary care provider, any specialists you may be seeing, your therapist, and any previous medication management prescriber.



The purpose of the ROI form is to allow us to obtain relevant medical information from your current and previous healthcare providers.

  • Communication

    We will communicate with you via the client portal. It this is not the right method for you, then please advise us of your communication preferences. 


    Please allow 48-72 business hours for a reply to your phone call, email or message. To ensure HIPAA compliance, we utilize Spruce for phone, email and text. 


    Please be sure to download the Spruce app.

  • Engagement

    Active participation in your treatment is crucial. Attend scheduled appointments, follow treatment recommendations, and communicate openly with your provider.

  • Patient Feedback

    We are committed to providing the best care possible. Your feedback is important to us. 


    Please let us know how we can improve our services by leaving us a review or speaking directly with our staff.

  • Respect

    We ask that all patients treat our staff and other patients with respect. Any form of abusive or inappropriate behavior will not be tolerated and may result in termination of services.
